- To provide an environment where residents can feel safe and well and do more of what matters to them
- To help people to build personal resilience in self-managing their own health and wellbeing
- Respect and promote people’s rights to privacy, dignity and choice
- Work with people who use our service to ensure that there are a range of meaningful activities available that will support their wellbeing and engagement with the local and wider community
- Complete a “wellbeing assessment” with individuals to identify any additional support that may be needed and review this regularly
- Advise on areas such as health, diet, exercise and social isolation
- Help / signpost people to welfare benefits and income maximisation
- Respond to calls for assistance and help from people who use our service and staff members as required, taking necessary actions in line with the Schemes procedures in response to emergency situations
- Complete a daily telephone call or visit to all people who require this service, recording all relevant details and sharing any concerns with the care and wellbeing staff or home care team
- To record each visit to individual properties on relevant documentation. This must be done in line with the organisations confidentiality policy

- To meet the care & support needs of service users in a way that respects the dignity of the individual and promotes independence.
- Provide care and support to service users to ensure their personal, social, spiritual and emotional and support needs are met, in line with their individual care & support plans
- Assist service users with mobility problems and other physical disabilities, including the use and care of aids and personal equipment. Where there is a need for service users to be moved, then reference must be made to the care plan/risk assessment to ensure that the manoeuvre can be carried our safely.
- To record each visit to service users’ flats on relevant documentation. To make verbal and written reports of care & support input provided to each service user. This must be done in line with the organisations confidentiality policy
- Be aware of and adhere to the organisation’s policies contained in the Employment Policies and Procedures Manual
